Try to relieve your wife's concerns. I was scared of that little plane at first too, however, I've been to AC, 4 times now and have even flown over to the mainland and back just to go inland. The poster who said the planes are just like a taxi is correct. It is a lovely flight, if you just let yourself relax enough to enjoy it. Quite fabulous, actually. The pilots just fly back and forth all day long everyday. No worries.
AC is the only place I've traveled that I have never become ill. I've had tourista all over the world but *never* there. I brush my teeth with the water but buy bottled to drink though I have been known to drink the tap water too when I was out of bottled. No problems.
I've never had a botfly or even too many skeeter bites. I've had some nasty bouts with sandfleas but I take care of that now with the deet (Deep Woods Off in the small white pump bottle with the green top).
As for crime, well, I've had no trouble at all. I've heard a story or two but there's more crime here at home. Just take the usual precautions that any smart person would take anywhere and you should be just fine. I've walked all over a good portion of San Pedro by myself in the day as well as the middle of the night, if that is any reassurance.
The only real thing that I would caution your wife to do, is to take tissue with her in her purse when you guys go out. There's always a good chance there won't be any tissue in the ladies room.
